If you have applied for disability benefits and your claim was denied, you will want to file a request for reconsideration, which is referred to as form SSA-561.
Request for Reconsideration
The Social Security Administration (SSA) should send you a copy of this form when you are notified of the denial, but you can also download a copy from the SSA website under the forms section. There is no charge for the form, and you can download it and print it at home.
Referred to as an appeal, SSA – 561 is done by mail. If your claim was denied for medical reasons, you can file an appeal online. If you don’t want to start the appeal online, you can submit the form.
Along with the form SSA-561, you will need to submit two other forms – SSA-3441, Disability Report – Appeal and SSA-827, Authorization to Disclose Information to the Social Security Administration.
